Exemption Information
The Douglas County Health Public Health Order (effective 10/9/21) allows parents to sign mask exemption forms for their children if they deem masks to have negative physical or mental effects on their children. It also allows adults in a school setting, including staff and parent volunteers, to file the same exemption for themselves.

Instructions for Filing an Exemption
There are three steps to filing an exemption.
1) Download the TEMPLATE first and fill it out completely.
- If you volunteer regularly in the building and want to file a mask exemption, please fill out the volunteer exemption form.
- Please note: If you are a staff member and a parent volunteer and want to file an exemption, you need only fill out the staff exemption form.
2) Sign your form(s) physically. Electronic signatures will NOT be accepted.
3) Submit your form one of two ways:
- Submit electronically by filling out the GOOGLE FORM below.
- At the bottom of the Google form, you can upload a photo, pdf or word document version of the SIGNED TEMPLATE.
OR
- Submit in person
- If you do not have a gmail account: Access to Google Forms uploads requires a gmail account. If you do not have a gmail account, please bring your form(s) into the school and drop them off at the front desk .
- If you are unable to upload your template: If you do not have the ability to upload a photo, pdf or word document version of the signed template, please bring your form(s) into the front desk
Because of the important nature of these documents, please do NOT send them in with your children! We ask you to personally bring them in. You can drop them off during school hours or bring them to Parent/Teacher Conferences on Oct. 12 & 14.
